Advance Care Planning

Plan your future and have your wishes heard if you are unable to speak for yourself. Filling in an Advance Care Directive enable you plan ahead and decide what kind of medical and personal care you would like.

“We recommend an advanced care directive for anyone aged 18 and older. It is important because not only does it say what you don’t want, it also says what you do want. By filling it in you remove the pressure on family and friends if you can no longer express your wishes”, says MCCI’s Gail Jugow.

“The document includes medical preferences and your spiritual and cultural needs; you can state that you don’t want to have certain medications or invasive therapy (artificial breathing for example), and that you do want fresh flowers and be wheeled out into the garden every day, and then family, friends and the medical service can follow up.”

Download the information booklet and fill in the form, give it to your family and Doctors, and you can also upload it online to My Health Record:
