Plan your future and have your wishes heard. Filling in an Advance Care Directive enables you plan ahead and decide what kind of medical and personal care you would like if you can no longer speak for yourself.

“We recommend an Advance Care Directive for anyone aged 18 and older. It is important because not only does it say what you don’t want, it also says what you do want. By filling it in you remove the pressure on family and friends if you can no longer express your wishes”, says MCCI’s Selen.

“The document includes medical preferences and your spiritual and cultural needs; you can state that you don’t want to have certain medications or invasive therapy (artificial breathing for example), and that you do want fresh flowers and be wheeled out into the garden every day, and then family, friends and the medical service can follow up.”

Download and fill in the form, give it to your family and Doctors. You can also upload it online to My Health Record.
